Margherita, an eighty-year-old actress with a glorious past, lives autonomously in Rome until she is struck by ALS. The progressive disease forces her into immobility and to give up an imminent Hollywood film, sparking anger and frustration. Her son, Enea, a musician facing an existential and marital crisis, calls for help from his father, Antonio, Margherita’s former partner and a director. Despite forty years of separation and initial friction, Antonio moves in to assist Margherita, rekindling a love that had never faded. The illness leads to an intense reconciliation: Enea resolves his family conflicts, while Antonio and Margherita rediscover their lost intimacy. Faced with irreversible deterioration, Margherita asks Antonio to help put an end to her suffering.

In a gesture of immense love, Antonio agrees to Margherita’s wish, turning off the machines while she watches one of his old films. Antonio leaves the house and heads to the sea, in a painful yet dignified farewell.
